Services
Where we can help
Different levels of support depending on what you need, from getting your NDIS plan up and running through to specialist coordination for more complex situations.
Support Coordination
Level 2Whether you’re brand new to the NDIS or you’ve been in the system for years, we provide ongoing, hands-on support tailored to where you’re at.
We coordinate across multiple providers, problem-solve when things change, and keep your plan working toward your goals, all while keeping up with the NDIS rules changing all the time so you don’t have to. From plan reviews and NDIA communications to the day-to-day complexity of your unique disability journey, you won’t be navigating any of it alone.
- Coordinating across multiple providers so your supports work together
- Problem-solving quickly when circumstances or supports change
- Preparing for plan reviews and handling NDIA communications
- Keeping your plan on track toward your goals, day to day
Specialist Support Coordination
Level 3Similar to Level 2, but for participants funded at a complex needs level. We help you navigate situations that need a steadier, more specialised hand.
This level of coordination is for participants whose situations involve added complexity, things like housing instability, hospital admissions and discharge planning, several conditions or disabilities at once, or a breakdown in their support network. We work closely with your broader support network and any relevant services to reduce that complexity and keep your supports stable and coordinated when things are volatile.
- Navigating housing instability, hospital admissions, discharge planning and other complex situations
- Coordinating support when several conditions or disabilities overlap
- Rebuilding stability when a support network breaks down
- Working closely with your broader support network, including allied health and mental health teams
- Close, hands-on coordination during periods of crisis or significant risk
Psychosocial Recovery Coaching
Capacity BuildingRecovery isn’t a destination, it’s a journey, and it looks different for everyone. We walk alongside you through the ups and downs of that journey.
We help you build everyday skills, connect with the right services, and work toward a life that feels full and meaningful. Backed by lived experience and a background in psychology, our support is empathetic, grounded, and recovery-oriented, whether you’re just starting out or navigating a difficult stretch of your journey.
- Setting goals together and building everyday skills at your own pace
- A wellbeing plan for harder days, with mental health and crisis contacts close at hand
- Connecting you with the right services and supports
- Recovery-oriented coaching grounded in lived experience and psychology
Self-Management and Plan Management
Capacity BuildingHelp to take charge of your own plan, from reading the paperwork to dealing with providers on your terms.
Some people want to manage more of their NDIS plan themselves, or need a hand getting their head around how it all works. We work through it with you, step by step, until you feel confident doing it on your own.
- Making sense of invoices and service agreements
- Keeping track of your funding and what you’ve spent
- Speaking up and working confidently with providers
- Setting up simple systems that keep your supports organised
The extra work we put in
-
Risk assessment and management plans
When there are risks to think about, we put a plan in place so everyone supporting you knows what to watch for and what to do.
-
A support plan written for you
We write a support plan around your needs and goals, so every provider is working from the same page.
-
Community and mainstream programs
We look past NDIS funded services to community and mainstream programs that fit what you need.
-
Options beyond the usual
If the usual approach isn’t right for you, we look for another. Not everyone wants traditional talk therapy, for example, and there are other ways to get psychological support.
-
Bringing your team together
We bring the people supporting you together, across health, disability and other systems, so everyone is working toward the same goals.
Which service is in my plan?
-
Look under the “Capacity Building Supports” section of your NDIS plan.
-
In that section, look for “Support Coordination” or “Psychosocial Recovery Coaching” funding.
-
We’re only able to support those with self-managed or plan-managed support coordination funding.
-
If it’s not clear, you can contact the NDIA on 1800 800 110, or get in touch and we can help you make sense of it.
